Travel Reimbursement Details for any/all meetings:
After each meeting, please email a PDF of your receipts (one per page) and a completed copy of this form to Donna McMinn at dlp18@psu.edu and Suzy Unger at sgunger1@geisinger.edu. We will then process your reimbursement through Penn State. A check will be made out and mailed to you personally from Penn State.
Deadlines:
Meeting 1 – Submit all receipts by Friday 4/22
Meeting 2 – Submit all receipts by Friday 5/6
Meeting 3 – Submit all receipts by Wednesday 5/25
Reimbursable expenses:
Airfare (lowest available commercial economy non-refundable fare that meets the business purpose of the trip from the nearest airport)
Taxi/Shuttle
Hotel (group room rate/tax/internet fees only, no room service/incidentals)
Bag Check Fee (1 each way)
Mileage (up to 750 miles round trip)
*Any meals not provided by the conference will be reimbursed as per diems, so meal/coffee/snack receipts are not required.
